- Questionnaire – Client Information Questionnaire (PDF)
- Instructions
- (a) Right click the file above and press ‘Save As’ and save or download to your computer
- (b) Right click on the downloaded file and choose to Open With > Adobe Reader;
- (c) Complete all of the required questions;
- (d) When complete, save and return the Questionnaire to us by following the instructions at the end of the Questionnaire.
Please note that you must use Adobe Reader or Acrobat to open the Questionnaire. Other PDF readers are not capable of opening the Questionnaire. Adobe Reader can be obtained from the Adobe website: http://get.adobe.com/reader/
If you are having a problem opening the Questionnaire please:
(i) If you are using a PC, make sure that you have saved the item to your desktop and that you right click the file and choose Open With > Adobe Reader;
(ii) If you are using a MAC, make sure that you have saved the item to your computer and that you ctrl click the file and choose Open With > Adobe Reader;
(iii) If you are using a non-windows tablet or phone – please use a PC, MAC or Windows tablet.
